Clinical Reassessment of Posture, Seating and Equipment

A fresh clinical review when the person, their equipment, care arrangements or environment has changed, or when an existing solution is no longer meeting its intended outcomes.

Why reassessment matters

Posture and equipment needs change over time. A solution that was clinically appropriate when supplied may become less effective as the person grows, recovers, experiences deterioration, gains or loses weight, develops pain, changes activity or moves to a different environment.

Reassessment looks at the present situation rather than assuming that previous findings still apply. It can clarify whether adjustment, repair, replacement, a new trial, changes to support or input from another part of the multidisciplinary team should be considered.

Reasons to request a review

A review can be planned or prompted by a particular concern. The urgency and appropriate professional response depend on the circumstances, especially where pain, skin integrity or sudden functional change is involved.

  • Growth, weight change or altered body shape
  • Deterioration, recovery or a new diagnosis
  • Pain, discomfort or reduced sitting tolerance
  • Pressure risk or skin integrity concerns
  • New or changed equipment
  • Altered mobility, function, care needs or environment

Reviewing outcomes and current equipment

The clinician will revisit the goals of the original intervention and ask how the equipment or posture plan is working in daily life. Current posture, movement, comfort, function, transfers, pressure information, care delivery and equipment condition may all be relevant.

Where possible, useful records from previous assessments, specifications, repairs and treating teams help establish what has changed. Reassessment may still identify new findings that require a different clinical response.

Clear next steps

Not every reassessment leads to complete replacement. Sometimes an adjustment, component change, repair, updated guidance or focused equipment trial is the proportionate next step. In other cases, a more comprehensive postural or wheelchair seating assessment is needed.

We can document findings and liaise with the individual, family, carers, referrer, commissioner, treating clinicians and suppliers as agreed. This supports transparent decisions and avoids changes being made without understanding their effect on the whole system.

Follow-up after changes

If equipment is modified or replaced, final fitting and follow-up can confirm whether intended outcomes are being achieved and whether further adjustment is required. The review interval should reflect clinical need rather than a fixed timetable applied to everyone.

Sudden deterioration, acute pain or urgent skin concerns require appropriate healthcare advice and should not wait for a routine equipment reassessment.
